Geriatric Care Management
Your Home Companion provides geriatric care management. Geriatric care management includes providing information, support and services to improve the quality of life for our clients. Geriatric care management is just another one of our services that we provide to our clients in hopes of alleviating stress to all involved – family members, caregivers and even the client themselves.
Our geriatric care management services are non-medical. Our Care Managers are Nurses – RNs and LPNs who have experience working with seniors and the elderly. They have the knowledge and experience to be an advocate for our client’s health and well being. Our Care Managers will first perform an in-home assessment (this assessment is not just limited to our client’s home). Our in-home care assessment includes information regarding the client’s:
- Medical history such as illnesses, surgeries, and medications.
- Daily activities to determine level of care
- Current living situation and completion of a home safety check
- Financial situation to determine if they are eligible for any types of benefits and/or assistance.
From this information we determine a plan of care and share with you our recommendations and goals. We will discuss the services that may be needed and closely monitor and adjust the services on an as needed basis.
Other services our Care Managers provide include:
- Acting as a liaison for the family to stream line communication between family members and the client and/or other professionals
- Acting as an advocate for our client’s in a nursing facility, hospital or at home
- Assisting with long-term placement if needed
- Supervising repairs, maintenance and home modifications if needed
- Managing finances if necessary
